Our team:
Medical Director: The Medical Director (MD) is responsible for developing and overseeing a comprehensive plan of care and providing medical guidance for the team while working alongside the patient’s primary care physician.
Nurse Practitioners: They provide in-person evaluation and assessment visits to ensure eligibility and may visit to evaluate or treat a specific issue.
Registered Nurse: We have skilled nurses who are available 24 hours a day and are specifically trained to handle pain and symptom management.
Social Worker: Our social workers assist families with emotional and psychosocial needs and provide counseling services and community referrals.
Certified Home Health Aide and Homemaker: We assist with personal care needs, including assistance with daily living and hygiene.
Chaplain: Our chaplain provides spiritual support (if needed) to the patient’s families
Volunteer: Our volunteers provide companionship and assistance through visits, writing letters, legacy building, reading, and/or shopping.
Consultant Pharmacist: They manage the pharmaceutical care of hospice patients and provide drug information and education to the hospice staff, patients, and their families. A pharmacist is available 24/7.
Bereavement Coordinator: Our Bereavement Coordinator counsels the families by providing an initial bereavement assessment and offers support through the grief experience after the patient dies. They maintain contact with family and loved ones throughout the patient’s illness and for up to 12 months following the end of life.
Medications, Supplies, and Equipment: We provide all medications related to the patient’s diagnosis that are 100% covered and will be delivered to your home. The hospice also provides incontinence supplies. Medical equipment is covered 100% by the hospice
Transportation: Non-emergent transportation is provided to and from a contracted facility for inpatient and respite levels of care.
