Eligibility requirements are determined based on the clinical judgment of the Primary Care Physician and the Hospice Medical Director that certify that the patient has a limited life expectancy. The patient’s Primary Physician authorizes hospice care. The focus is on physical, emotional and spiritual comfort rather than curative measures.
